Okhunov general-surgery@mail.ru
Background. In recent years, the use of cytokines and various growth factors in the treatment of chronic wounds in patients with diabetes mellitus has been of great interest. This manuscript is devoted to this particular problem, which made it possible to improve the results of treatment. Material and methods. The results of the treatment of 72 patients with type 2 diabetes mellitus with diabetic foot syndrome are presented, consisting of two groups: the main group (38 patients who, along with the standard treatment of diabetic foot syndrome, underwent local applications with a solution of the immunomodulator Superlymph according to the generally accepted scheme) and the comparison group (34 patients who used traditional treatment for this pathology). Clinical, laboratory and instrumental research methods available in clinical settings were used. Results and conclusions. A change in the local cytokine status in the form of a decrease in anti-inflammatory and an increase in pro-inflammatory cytokines is characteristic of patients with diabetic foot syndrome. The levels of cytokines of wound discharge exudate depend on the duration of diabetes mellitus, and the level of glycated haemoglobin and leukocytosis and do not depend on the form of diabetic foot syndrome. The use of Superlymph in the complex therapy of patients with diabetic foot syndrome contributes to the normalization of the immune status, which in turn ensures effective healing of ulcerative foot defects in diabetic foot syndrome.
2023-10-04T00:00:00+00:00 Copyright (c) 2024 https://journals.tma.uz/index.php/cajm/article/view/678 TREATMENT RESULTS EVALUATION OF PATIENTS WITH LOWER LIMB ARTERIAL THROMBOSIS AFTER CORONAVIRUS INFECTION 2024-02-18T18:26:05+00:00 A.A. Tracheal trauma is a time-dependent, high-risk acute condition that can adversely affect the patient's ability to maintain a patent airway. Optimal initial stabilization requires a team approach that includes emergency medicine, ENT, and ancillary services including respiratory therapy. Curative treatment, which involves emergency surgery, has been shown to lead to positive outcomes, as opposed to watchful waiting, which is associated with worse outcomes. The review article provides data on the diagnosis, methods and results of treatment of patients with acute trauma to the trachea and main bronchi. It is noted that only timely diagnosis, adequate surgical and anesthetic care can ensure a favorable treatment outcome.
2023-10-04T00:00:00+00:00 Copyright (c) 2024 https://journals.tma.uz/index.php/cajm/article/view/682 METHOD AND IMPLEMENTATION OF SURGICAL CORRECTION FOR SPINE DEFORMATIONS 2024-02-18T18:47:00+00:00 Fatkhulla R. Haydarova y_ras@mail.ru Komiljon K. Safarov Safarov@mail.ru
Parkinson’s disease (PD) is a common neurodegenerative disease typified by a movement disorder consisting of bradykinesia, rest tremor, rigidity, and postural instability. Treatment options for PD are limited, with most of the current approaches based on restoration of dopaminergic tone in the striatum. However, these do not alter disease course and do not treat the non-dopamine-dependent features of PD such as freezing of gait, cognitive impairment, and other non-motor features of the disorder, which often have the greatest impact on quality of life. As understanding of PD pathogenesis grows, novel therapeutic avenues are emerging. These include treatments that aim to control the symptoms of PD without the problematic side effects seen with currently available treatments and those that are aimed towards slowing pathology, reducing neuronal loss, and attenuating disease course. In this latter regard, there has been much interest in drug repurposing (the use of established drugs for a new indication), with many drugs being reported to affect PD-relevant intracellular processes. This approach offers an expedited route to the clinic, given that pharmacokinetic and safety data are potentially already available. In terms of better symptomatic therapies that are also regenerative, gene therapies and cell-based treatments are beginning to enter clinical trials, and developments in other neurosurgical strategies such as more nuanced deep brain stimulation approaches mean that the landscape of PD treatment is likely to evolve considerably over the coming years. In this review, we provide an overview of the novel therapeutic approaches that are close to, or are already in, clinical trials.
2023-10-04T00:00:00+00:00 Copyright (c) 2023 Central Asian Journal of Medicine https://journals.tma.uz/index.php/cajm/article/view/696 COMPARATIVE CHARACTERISTICS OF ANTHROPOMETRIC INDICATORS IN ADOLESCENTS WITH DOWN SYNDROME (LITERATURE REVIEW) 2024-02-18T20:22:34+00:00 Shukhrat J. Redox dysregulation and a dyshomeostasis of inflammation arise from, and result in, cellular aberrations and pathological conditions, which lead to cardiovascular diseases. Despite years of intensive research, there is still no safe and effective method for their prevention and treatment. Recently, molecular hydrogen has been investigated in preclinical and clinical studies on various diseases associated with oxidative and inflammatory stress such as radiation-induced heart disease, ischemia-reperfusion injury, myocardial and brain infarction, storage of the heart, heart transplantation, etc. Hydrogen is primarily administered via inhalation, drinking hydrogen-rich water, or injection of hydrogen-rich saline. It favorably modulates signal transduction and gene expression resulting in suppression of proinflammatory cytokines, excess ROS production, and in the activation of the Nrf2 antioxidant transcription factor. Although H2 appears to be an important biological molecule with anti-oxidant, anti-inflammatory, and anti-apoptotic effects, the exact mechanisms of action remain elusive. There is no reported clinical toxicity; however, some data suggests that H2 has a mild hormetic-like effect, which likely mediate some of its benefits. The mechanistic data, coupled with the pre-clinical and clinical studies, suggest that H2 may be useful for ROS/inflammation-induced cardiotoxicity and other conditions.
2023-10-04T00:00:00+00:00 Copyright (c) 2024 https://journals.tma.uz/index.php/cajm/article/view/699 THE NEGATIVE EFFECT OF LONG-TERM CHEMOTHERAPY ON HEART FUNCTION 2024-02-18T20:38:40+00:00 Jakhongir A. Typically, the immediate side effects of chemotherapy may persist for several days or weeks after treatment and may include fatigue, nausea and vomiting, hair loss, mouth ulcers, and changes in appetite or taste. These side effects can be managed with medications, dietary changes, and other supportive treatments, and they usually go away over time. There may also be long-term side effects of chemotherapy, such as nerve damage, hearing loss and an increased risk of infection, which may persist for months or years after treatment. The prevalence rates for patients with bronchial asthma among the population are 30% mild , 50% moderate, and 20% severe bronchial asthma patients [5].

Bronchial asthma causes disability and major economic damage in the population. High rates of bronchial asthma have been reported in Uzbekistan. In 2019-2022, its prevalence in the Fergana region was studied, accounting for 7.2% [1].

Bronchial asthma has a single immune-inflammatory form that also affects the organs of the oral mucosa and the respiratory tract. In patients with bronchial asthma, the oral cavity has a significant effect on the decrease in the protective properties of the mucous membrane, which creates unfavorable conditions for the hard tissues of the tooth and periodont tissue, increases the effect of microflora and other pathogenic factors. There is information about the presence of disorders in the immune system in patients with bronchial asthma who are prone to the occurrence of inflammatory periodontal diseases [2,4,9].

Both an attack of bronchial obstruction and a relatively calm period of the disease require the appointment of medications, in particular glucocorticosteroids. At the same time, along with a positive effect, these drugs lead to a decrease in the natural protective barrier of the oral mucosa and dysfunction of immune defense systems. This can significantly complicate adequate therapy of the disease, contributing to the development or progression of local and general inflammatory diseases [11,12].

With the beginning of the use of glucocorticosteroids, the results of preventive and maintenance therapy of bronchial asthma have improved significantly, but currently the effect of these drugs on the condition of the oral mucosa is little studied. Studies have not been carried out in patients with bronchial asthma, taking into account the condition of the oral cavity, the clinical and pathogenetic variant, duration and methods of this disease [4].
2023-10-04T00:00:00+00:00 Copyright (c) 2023 Central Asian Journal of Medicine https://journals.tma.uz/index.php/cajm/article/view/711 MORPHOLOGICAL FEATURES OF MESENTERIAL LYMPH NODES IN EXPERIMENTAL HYPOTHYROIDIS IN THE MOTHER 2024-02-18T21:36:00+00:00 Feruza Kh. After establishing a stable decrease in the concentration of free thyroxine in the blood serum, the females were fertilized by healthy males. During pregnancy and lactation, females continued to be administered a maintenance dose of the drug.

At 3, 7, 14, 21 and 30 days after birth, the development and formation of MLN offspring obtained from females with experimental hypothyroidism were morphologically studied.

It has been established that in offspring born under conditions of maternal hypothyroidism, the time parameters for the formation of structural and functional zones of MLN, especially its T-dependent zones, significantly lag behind the control.
2023-10-04T00:00:00+00:00 Copyright (c) 2024 https://journals.tma.uz/index.php/cajm/article/view/712 CLINICAL AND LABORATORY MANIFESTATIONS OF TUBERCULOUS SPONDYLITIS 2024-02-18T21:40:19+00:00 Abduvakhob S. Yuldasheva maxfira@mail.ru Yulduz G. Rasul-Zade y_ras@mail.ru
Cryopreservation of gametes and embryos is one of the most pressing problems of modern reproductive medicine. A comparative analysis of the effectiveness of the use of natural and frozen materials was carried out in the embryological laboratory. With the help of carriers of the closed (n = 598) and open (n = 665) types, 1263 molten embryos were transferred. Most of the indicators were good when using open carriers. The data obtained allows us to conclude that vitrification of morphologically normal gametes does not reduce the effectiveness of cryodastures and maintains their reproductive status. The use of open-type carriers to vitrify and store embryos demonstrates high levels of embryo retention after cryoconservation, with rates of miscarriage and low loss. It is assumed that this is due to the direct contact of the biological object (embryo) with liquid nitrogen and high freezing levels, which can be a decisive factor in the successful determination of vitrification in open carriers.
2023-10-04T00:00:00+00:00 Copyright (c) 2024 https://journals.tma.uz/index.php/cajm/article/view/715 DEVELOPMENT OF AN ALGORITHM FOR DETECTING DENTAL DISEASES AND PROVIDING DENTAL CARE TO CHEMICAL INDUSTRY WORKERS 2024-02-18T22:41:01+00:00 Sunatullo A. The relationship of the high prevalence of non-carious lesions of the teeth and oral mucosa with such industrial processes as oil and gas extraction, metallurgy, chemical production, bakery and confectionery production has been proven. Dental health contributes to the preservation of the ability to work of an important part of the non–population of the country - workers of industrial enterprises, especially since a number of studies convincingly reveal the role of the dental system in the general state of the body. [2,6,8,]

The presence of high risks of occupational diseases among workers in industry has been proven. This is due to the entry of compounds of elements into the human body through the oral cavity. Industrial workers are characterized by a high prevalence of dental caries and inflammatory periodontal diseases.
2023-10-04T00:00:00+00:00 Copyright (c) 2024 https://journals.tma.uz/index.php/cajm/article/view/716 THE COURSE OF DIABETIC POLYNEUROPATHY IN THE COMORBID DEFICITIES OF THE BODY (CLINICAL CASE) 2024-02-18T22:44:55+00:00 Nodir K. Diabetic polyneuropathy (DPN) is the most common complication of diabetes mellitus (DM) in the peripheral nervous system. A common clinical form of DPN is a distal symmetrical, mainly sensory, polyneuropathy. The basis of the pathogenesis of DPN is chronic hyperglycemia, which leads to the accumulation of excess intermediate products in various organs and tissues, including damage to the vasa nervorum, which is involved in the blood supply of peripheral nerves. This article examines the deficiency states of the body in the case of diabetic and comorbid polyneuropathy.
2023-10-04T00:00:00+00:00 Copyright (c) 2024 https://journals.tma.uz/index.php/cajm/article/view/717 CHANGES IN THE HISTOARCHITECTONICS OF THE KIDNEYS IN POSTNATAL ONTOGENESIS DURING ALCOHOL INTOXICATION 2024-02-18T22:48:33+00:00 Shukhrat J. The babies were divided into 2 groups. Group 1 - the main group of 68 infants (of which 42 are male and 26 are female) and group 2 - the control group of 66 infants (of which 41 are male and 26 are female).

The studied changes and their systematization lead to a decrease in maxillofacial pathologies, as well as growth and development of the definition, which contributes to the non-hormonal development of the child. The introduction of the obtained data into practical healthcare will reduce the proportion of maxillofacial anomalies. For the first time, the method of using early orthodontic treatment of children with CCLP, carried out with the help of the "Dental Obturator for orthodontic treatment of children with CCLP" was scientifically substantiated.
2023-10-04T00:00:00+00:00 Copyright (c) 2024
